Creative Care Consultants assists the older adult and their family through
the self-determination process by gathering complete medical, social, and
environmental information and
presenting available options and choices.
Assessment
By evaluating the older person in their present environment, we can best
determine their
functional abilities, medical condition, mental status, family involvement
and financial resources. We utilize a social
services/nurse team approach to assure that all of their physical, mental,
emotional and environmental needs are addressed. At Creative Care
Consultants we make the wishes, values and preferred environment of the
older adult part of our recommendation. We evaluate the ability to live
alone and provide resources and/or referrals that enable the older adult to stay at home as
long as possible.
Care Planning
After assessing the need for short and long term care, in-home assistance,
the most appropriate living situation, the financial resources available
and the level of responsibility the family is able to take, we provide you
with a variety of options, along with a cost analysis of each one. We
inform you of all available resources, facilities and programs and help you
develop a care plan that best fits your needs, while including the older adult
in the decision-making process as much as possible.
Care Management
After we have worked with you to develop the best possible care plan to meet
the specific needs of the older adult, we are there to assure that the care
plan is put into place. Creative Care Consultants continues to monitor
the individual's health and well-being, and provide mediation when needed. We
are able to assist with routine services, including bill paying, transportation
needs, coordination of medical care and medical claims filing. Many out-of-town
families utilize our services on an on-going basis. We are able to take on
some of the responsibilities that they would have if they were available,
such as escorting the older person to medical appointments, monitoring their
care and helping to handle daily money management. Families are kept apprised
of their loved ones' status through written monthly care management updates.
Crisis Intervention
Creative Care Consultants has a staff member on call at all times in the
event that one of our clients has an emergency. If a crisis arises, we
are available to react on an immediate basis to reassess the situation,
(in or out of the hospital), communicate with physicians, arrange for
post-hospital care providers, and identify the need for short and long term care.
Our expertise in working with older
adults enables us to meet challenges as they arise and make necessary
recommendations based on the individual's needs.
